Curriculum Programmes
1. RSE Programme — I consent to my child's participation in the Relationship and Sexuality Education (RSE) Programme. This compulsory programme forms part of the Primary School Curriculum and addresses emotions and relationships in an age-appropriate way.
2. Stay Safe Programme — I consent to my child's participation in the Stay Safe Programme. This programme forms part of the Primary School Curriculum, is delivered every two years, and teaches children personal safety strategies in an age-appropriate manner.
Assessment and Educational Support
3. Standardised Screening Tests — I consent to my child participating in standardised screening tests carried out in school from Infants to 6th Class. These tests are designed to monitor academic progress and identify areas where additional support may be beneficial.
4. Diagnostic Testing — I consent to my child participating in individual diagnostic testing carried out by teachers as needed, to support my child's educational development and inform planning.
5. Special Education Teacher (SET) Support — I consent to my child receiving additional learning support from the school's Special Education Teacher (SET). This support may be provided on an individual, paired, or group basis for a designated period, as agreed by the class and support teachers.
Activities
6. Supervised Local Walks — I consent to my child participating in supervised local walks, such as visits to the community centre, church, or playground, where appropriate.
Photography and Media
7. School Website and Blog — I consent to my child's photograph or image being included in school-related activities, competitions, the school website, and the school blog.
8. School Newsletter — I consent to my child's photograph or image being included in the school newsletter.
9. Social Media — I consent to my child's photograph or image being posted to the school's social media accounts (including Facebook and Instagram).
Data Sharing
10. Sharing Information with Agencies — I consent to my child's and family's details (name, address, date of birth, etc.) being shared with relevant external agencies, such as the HSE (school nurse, doctor, dentist), where appropriate and in accordance with the school's Data Protection Policy.
Communications
11. Text-a-Parent Service — Parent / Guardian 1 — I consent to the school contacting Parent / Guardian 1 by text message with school-related updates and notifications.

Medical Emergency
13. Medical Emergency — In the event of an emergency or accident, a member of staff will use their discretion and bring my child to a Doctor or Hospital. Every effort will be made to contact me beforehand. I consent to a member of staff bringing my child to a Doctor or Hospital if a medical emergency arises.
Declaration
By signing below, I/we confirm that the consent choices indicated above are accurate and freely given. I/we understand that consent may be updated at any time by submitting a new form to the school office and that this submission supersedes all previous consent choices on file for the child named above. I/we acknowledge that this electronic signature has the same legal standing as a handwritten signature under the EU eIDAS Regulation (No 910/2014).
